How to determine break wear?

My car is hitting 30K miles and I am wondering how you determine break wear. Is there a minimum pad thickness?
Also, does one always need to change to rotors or can the first pad change be just pads and not rotors?

3-4mm is the minimum pad thickness.

You should rarely have to change rotors as long as you don't wait to long to change your pads. Most regular rotors will last 3-4 pad changes. Carbon Ceramic rotors are designed to be lifetime.
