Wondering if anyone else has seen this...has happened 2 or 3 times since I bought the car CPO last November with 9k miles.

I'll receive a phone call on my mobile (iPhone 4G) using bluetooth linked to PCM. PCM will switch from stereo to phone mode. I'll accept the call, have the conversation and then proceed to hang up -

And PCM won't hang up the phone. I'll try the button on the MF steering wheel, the **** on PCM, the touch screeen display - nothing.

I have to hang up the call on the phone itself.

Then PCM won't return to stereo mode. Shortly afterwards, especially if I'm in another call, PCM will glitch, then appear to reboot (screen goes blank, go through power-up procedure).

Several other observations:

1. Happened before and after MF steering wheel install
2. Happened with 2 different iPhone 4G's and across several iOS revisions

Is V1.40 the latest SW revision for PCM? Any thoughts or suggestions?

I don't know if 1.40 is the latest rev, but I do know that's not something I've ever seen with our 2009. Purely offhand, I'd say it's a problem with the switch matrix. That is, hardware rather than software. But the real answer is that it's a warranty issue.
