Clutch Slip

A couple of time over the past couple of weeks during hard acceleration I have felt my clutch slip between 1st and 2nd. The situation occurred during an attempt of a super-smooth shift (as my wife was in the car). I put the stick into second and smoothly push the gas pedal to the floor while smoothly letting out the clutch. After the clutch is fully released, it seems like there is a little slippage for a fraction of a second before it locks-up.

Is this another reason for not letting my wife in the car?

Maybe it's just the way you described it, but it sounds likle you're slipping the clutch during the 1-2 shift.

Being smooth with shifts does not mean being slow with the clutch release.
