Forget the insurance claim, you'll end up paying for it in the long run with increased premiums. Pay for it out of pocket and file a claim against the city/municipality. There's a good chance that manhole cover fell off a state vehicle. Depending on your state, you might even be able to file in small claims court. Just make sure you look up the rules for suing the state. They usually have some very specific requirements and time constraints. A little bit of time and research could easily get you reimbursed without needing to hire an attorney.
Just to be clear, this was resolved with my insurance company with no rate increases.

As for HRE wheels, I trust that they're quality items. This was a pretty specific impact with an uncommon piece of road debris. I don't expect them to be indestructible. At best the wheel would have been massively bent instead of broken and required replacement anyhow.

I don't see how they could try it any other way - stationary objects fall under comprehensive, moving stuff (crap falling from a truck etc) is considered collision.
