If this condition persists on every start up ( just on one side) it can be an indication of a scored cylinder/piston on bank 2 cylinder 4-5-6. I would have someone look into the cylinders on bank 2 with a borescope to check for damage .I hope this is not the case but I have seen it on quite a few m/96 3.4 engines on bank 2. If scoring is significant it will usually be accompanied by a ticking or tapping on that side of the engine. If this problem just started at the same time your drivability issue started then the smoke could be the result of A/F mixture being so rich that it washed down the cylinder and caused the smoke on start, if that's the case fixing the drivability issue will solve both problems.

See the left side blue smoke stream. Parked on level ground all night.

You need to see this. Is this condensation in oil filler tube? Does this help narrow issue?

Does the ticking/tapping always remain if there is scoring? Or does it go away after a few seconds of the engine running?

Are you talking about the 3 second clatter on start up? If so it is usually caused by the timing chain tensioners , there is 3 of them.

The ticking or tapping can come and go with changes in temperature as the piston swells and contracts as it warms and cools depending on how bad the scoring is, if the scoring is severe it may not go away at all. Also of course there are other things that can cause ticking or taping like lifters and can come and go with RPM and temperature, and be really hard to tell the difference. If you have a ticking or tapping on bank 2 of a m/96 engine I would look for scoring FIRST.

The oil foam/condensation on the filler tube is normal, It's because the air in the long fill tube does not get hot enough to burn off the moisture that from the air that circulates inside the engine....driving for short trips without getting to full operating temperature will cause this to worsen.....(my m/96 has done this for 158k miles) as long as the dipstick/oil shows NO sign of milk-shake your OK, just clean it every so often..Have you checked your cylinder for scoring yet??

Yes that's when she does it, but only occasionally. I figured it was probably a tensioner.
