Shifting problem

My cousin has a weird shifting problem. Car is manual 2001. He sometimes can’t shift downwards - it is an intermittent problem - hard to diagnose I guess. As he is driving let's say at 45mph he wants to shift the car from 3rd to 2nd. He can't get the shifter from 3rd into 2nd gear. Also, it happens from 4th to 3rd sometimes. And once it happened from 6th to 5th. He had short shifter fitted – but they rechecked and everything seems ok. However, then just like that the car is all fine again and allows all shifts. Then few days’ later problem reappears again... Any ideas?
