Sean so sorry this happened. Glad we got to get in there and at least see the picture rather than send the car off blindly. Hopefully we can get to the bottom of it soon. I will be in close contact with Robert at the local shop and anything else you need I am here bud.
Overtorque the nut and it can fatigue the shaft I'd expect. Not sure what else could be done to make that failure occur other than a defective shaft? If the blades bound up from FOD I would expect a shear below the wheel not above where the nut is...
I've never known an end user to try to do anything with that nut, it's set at the factory and balanced in place. The answers need to come from Tial IMO.
I truly hope that the pieces were all caught in his intercoolers and that he has a magnet on the sump. Actually, are those pieces even ferrous? Strip that filter out too! What a nightmare, fingers crossed man this could be a real nightmare

Loss of balance for whatever reason leads to some pretty awesome carnage when spinning at 150,000 rpm
Right...that was the first thing I noticed. My guess it that some debris hit the wheel, almost stopping it, hence forcing the nut off, but instead severing he shaft...
Got the turbos from the mechanic. Was getting ready to box them up when I heard loose metal bits. Rolled the failed turbo and all this came out. This is good as the more I find the less I need to track down in the engine. Shipping both turbos back to TiAL. Mechanic looked at bolt and first thing that came to his mind was that the nut was overtightened as well. We'll have to see what TiAL's ruling is now.
