Heel toe help, clutch issues.

Having injured my right ankle many times I find it almost impossible to hit the throttle pedal while breaking. My foot just does not bend that way. Are there any type of pedals or pedal extensions out there? I have even thought of making something that is activated by a lever/ring behind the steering wheel.
Another issue is the clutch. I have a lot of movement before the clutch starts to do anything, like more than half way. Can/should anything be done about this? Do I really need to push it all the way down like the book says? There are no funny noises when I short clutch shift, and I can shift a whole lot faster too. Any ideas?
