I start up and drive away, keep the RPMs below 4k until oil temp is 200.

it matters what the weather is like, for example when its winter in NY i warm up the car for about 3 min. During the summer its 20sec and its ready

Whether you warm it up at idle or driving slowly will always be debated, but one thing is for sure, you do NO HARM warming it up at idle and having the 6 speed in neutral with clutch engaged.

I've often wondered about this also.

I go directly from a very short country lane onto a long UPHILL on a one lane highway where I have to merge with traffic going 60 to 65. Try to keep it under 3500 rpm but need to get it moving or get rearended by a semi utilizing his momentum.

Water temp gets up quickly; oil temp takes a good while, I've found. Does sitting around my driveway revving gradually up and down help or hurt?

yeah same here. i just keep it real slow till everythings up 2 temp.

Same here. I back out of the garage within a few seconds & then it's a few minutes before I take get C2S above 3.5K rpms. The coolant temp is always creeping towards normal by then. A little common sense would suggest never redlining until all temps are at within normal range.
