It comes with the bluetooth module, yes. The Ipod cable you can find online for $20.

Is it a direct plug and play update for a current PCM 2.1 with BOSE?
What is the procedure for installation? Does all the fiber optic cables and speakers all over need to be replaced?

If your car is an 02+ I think all of the speakers would need to be replaced? It is not a plug and play. This is a Kenwood, your OEM unit is a BOSE. They do not share the same plugs.
