There is a pretty thorough TSB on the second gear issue, complete with photos of the damaged components and how PCNA will assess the root cause. There was an error in tranny assembly that caused the failure in some, but the vast majority is the fact that the 2nd gear is very susceptible to 'missed shifts' and a few 'grinds' will remove the material that holds the gear in place.

There is a 'fix' that involves changing a detent, that can reduce the issue- while they are there doing the work you might consider it- especially if yours is not bad.
