With this device, the "quiet" mode is the normal sound of the car, and then the "loud" mode makes it even louder, correct?

It opens the valves on the exhaust from a button that keeps them on or returns it to normal where the valves open after 3500 RPM. It's much easier and more rewarding using the paddles with valves open. They offer a version that defaults to valves open which is what I would recommend if I were to buy it again. I drive with the valves open until I am on the highway and then return the car to "normal".
