Not unlocked sorry but you can use it with AT&T, I am sure that there is a way to unlock it

I am not sure with CDMA, but if the problem is just with the unlocking thing, you won't have problem with it, there is an instruction somewhere online on how to unlock it

you cant use with verizon no sim card. You should be able to use them with any GSM provider (tmobile, cingular, att), providing it is unlocked.
